Community Arts Grant Report

Use this form to submit your final report for your approved Foundation Community Arts Grant. 

Final Report

Final report should be submitted no later than two (2) weeks after completion of approved project.

This report should include photographs (authorized for publication) and some form of ‘journal’ or progress evaluation, plus necessary receipts and financial accountability. Photographs are always helpful, but we must have consent forms and names included to use photos of students. Please include the same project, applicant names, and amount of grant.
